Justin Bogner b03fd12cef Replace "fallthrough" comments with LLVM_FALLTHROUGH ...

This is a mechanical change of comments in switches like fallthrough,
fall-through, or fall-thru to use the LLVM_FALLTHROUGH macro instead.
