Duncan P. N. Exon Smith 4abd1a0808 IR: MDNode => Value: Instruction::getAllMetadata() ...

Change `Instruction::getAllMetadata()` to modify a vector of `Value`
instead of `MDNode` and update call sites.  This is part of PR21433.
