David Blaikie 00be69ab5c Remove the CFGElement "Invalid" state. ...

Use Optional<CFG*> where invalid states were needed previously. In the one case
where that's not possible (beginAutomaticObjDtorsInsert) just use a dummy
CFGAutomaticObjDtor.
