Manman Ren f019cd62da Debug Info: LiveDebugVarible can remove DBG_VALUEs, make sure we emit them back. ...

RegisterCoalescer used to depend on LiveDebugVariable. LDV removes DBG_VALUEs
without emitting them at the end.

We fix this by removing LDV from RegisterCoalescer. Also add an assertion to
make sure we call emitDebugValues if DBG_VALUEs are removed at
runOnMachineFunction.

Partially revert r152770. That commit moved the default handlers to their own file. But it also did some refactoring. It is the latter that is being reverted. The refactoring had accidentally removed the required effect that the default unexpected_handler calls std::terminate(), which is a visible effect.
